Your First Visit

The information on this page provides a detailed overview of what to expect and how to plan for your first visit to Cogent Physical Rehabilitation Center.

Do the following before your first visit.

  • Go to our patient forms section of the website and complete the appropriate intake forms.

  • Please, arrive 5 to 10 minutes before your scheduled session begins to complete the registration process.

  • Bring your completed patient forms, your doctor’s note (if applicable), any relevant medical reports, and your insurance information.

  • Wear comfortable clothing for assessment and treatment programs such as exercises.

During Your Visit

The first part of your examination involves an interview during which your therapist will review your medical history and current complaints. After consenting to the assessment, the physiotherapist will ask you some questions such as:

  • What type of symptoms are you experiencing?

  • How long has your pain lasted?

  • How and when did your symptoms/pain begin?

  • What makes your symptoms worse?

  • What makes your symptoms better?

  • What activities are you presently experiencing difficulty with due to your complaints?

  • What treatments and therapies have you had in the past and what were the results?

  • What are your goals?

 

During your assessment, your therapist will complete some examinations such as measuring joint movement, muscle strength, posture, how you get in and out of bed, sit and stand and walk on level ground or stairs. Other appropriate clinical tests relevant to your complaints, will be completed. While some tests are designed to reproduce the pain for the purpose of confirming a diagnosis, clinical tests should not significantly increase your. If you feel a great deal of discomfort any point in time during your examination, make sure to communicate it with your therapist.

After the examination, your therapist will review the findings of the assessment and discuss a treatment plan (also known as a Plan of Care) to treat your condition and how this will help you to reach your goals. Please do ask if you have any questions at all. Research shows that patients who understand their conditions and play an active role in developing treatment goals achieve better results that are further sustained after their treatment.

If appropriate, we will initiate your treatment on the day of your initial consultation. Treatment may involve hands-on treatment like manual therapy, education about pain and swelling management or activity modifications, a brief exercise session or use of modalities such as electrical stimulation or acupuncture.

After Your Visit

Depending on the condition and the diagnosis, your treatment plan may involve 1-3 visits per week to our clinic. , Your therapist will be provide you some home exercise plan to ensure that you maintain the gains of in-clinic treatment sessions. Following the recommended exercise program is likely to help you to recover much faster and reduces the number of visits to the clinic.

Before you leave for the day, your therapist will arrange and confirm your follow up visits, coordinate your insurance billing and answer any questions you may have.
